Abstract
Introduction: Health equity requires coordinated action across disciplines. Methods: A mixed-methods study examined an interdisciplinary community programme. Results: Screening coverage rose by 28% and follow-up adherence improved. Conclusion: Interdisciplinary, community-anchored models meaningfully advance health equity.

Introduction

Persistent inequities in access and outcomes demand approaches that move beyond single-discipline interventions. This study evaluated an interdisciplinary community health programme addressing social determinants of health.

Methodology

A mixed-methods design combined service-utilisation data with qualitative interviews across three community clinics over nine months.

Results

Screening coverage increased by 28% and follow-up adherence improved substantially. Participants valued the continuity offered by a coordinated team.

Conclusion

Interdisciplinary, community-anchored care models are an effective strategy for advancing health equity.
